In accordance with Current Data Protection Legislation, patients may request to see their medical records. These requests are known as Subject Access Requests, and a request can be made verbally or in writing, however we always seek to fully identify patients, and obtain the relevant authority to release information, unless we are legally obliged by the authorities to do so. Completing the attached form will help us provide you with what you require quickly.

Your right to get your data corrected

You have the right to challenge the accuracy of personal data held about you by the surgery and ask for this to be corrected or deleted.  This is known as the “right to rectification”.  If your data is incomplete, you can ask for the surgery to complete it by adding more details.

If you spot something in your medical records that is not about you or notice any other errors, please complete the sheet below and return to the surgery.
